Community Services

MCPN Community Services

Community Services provides the right “supportive touch” at the right time and place. From consultations in the health centers, referrals to external resources, transitions of care, to community health programs, our team members are there to meet individuals and families where they are, and support them in achieving optimal health outcomes. Community Services team members address barriers to care such as housing, transportation, and food security, while supporting the understanding and management of chronic health conditions. Staff aims to provide “whole person care,” with the overarching goal of meeting people where they are and helping them reach their health goals.

Programs are created in conjunction with community partners, including community mental health centers, hospitals, state or local health departments, in order to address specific issues or health disparities within identified communities.

Accountable Care Collaborative

ACC is a Medicaid care coordination program designed to improve clients’ health and reduce costs.

Diabetes-Health Education Services

Supports patients in managing health through consultation on: diabetes, pre-diabetes, gestational diabetes, weight management, high cholesterol, high blood pressure, other chronic disease management.

Homeless Services Programs

Serve patients who are homeless or at risk of homelessness in the health center and community through outreach, health education, nurse visits, and linkages to health care and supportive services.

Patient Support

Patient education and linkage to resources within and outside of MCPN, including more intensive care coordination services.

Refugee Services Team

Supports newly arriving Refugees, asylees and other eligible individuals resettling in the State of Colorado with their medical screening and establishing a medical home.

MCPN Ryan White HIV Clinic

Free/low cost confidential medical, dental, and support services for individuals living with HIV/AIDS.

Women and Teens

Empowers patients to make informed choices about relationships, family planning, pregnancy, postpartum, and newborn care through educational programs, clinical care, and supportive services.
